Client Services Coordinator

Responsibilities/Duties:

  • Determine client eligibility for the program and services that best meets the client needs
  • Maintain accurate client records
  • Conduct in-person assessments and annual reassessments
  • Maintain a wellness call procedure and implement as needed
  • Establish a call checklist to identify areas of concern regarding a client’s wellbeing

Qualifications:

  • High School diploma minimum education with no less than five years qualifying experience
  • Undergraduate Degree in Social Work or Gerontology strongly recommended 
  • Experience working with the elderly in aging programs preferred
  • Excellent verbal and written communication skills a requirement                              
  • Basic knowledge and understanding of aging process a requirement
  • Ability to work collaboratively with colleagues
  • Organizational skills are critical
  • Computer skills and familiarity with Word, Excel, and Client Database software are critical      

Time Commitment:    During the hours of 8am-2:30pm
