Make a Difference in Our Community
Crossroads Healthcare Clinic, part of Community Action of Laramie County, is seeking a compassionate and experienced Family Nurse Practitioner (FNP) and/or Physician Assistant (PA) to join our dedicated team. This position plays a key role in delivering comprehensive primary healthcare services to individuals and families experiencing homelessness or living with low income in Laramie County. Care is provided in both a traditional clinic setting and through our mobile healthcare unit.

 

Key Responsibilities:

  • Deliver direct primary care to patients of all ages, including assessment, diagnosis, treatment, and prevention services
  • Manage acute and chronic conditions, perform physical exams, and administer immunizations
  • Order and interpret diagnostic tests, prescribe medications, and evaluate treatment outcomes
  • Provide health education and counseling to promote healthy behaviors
  • Maintain thorough documentation in the electronic health record (EHR) system
  • Collaborate with internal staff, community agencies, and other healthcare providers
  • Support and mentor students and assist in the training of clinical team members
  • Ensure compliance with HIPAA, OSHA, and clinic protocols

 

Qualifications:

  • Must be primary care certified
  • Licensed Family Nurse Practitioner or Physician Assistant in the State of Wyoming
  • Master’s degree from an accredited program
  • DEA license and current malpractice insurance
  • Minimum of one year of clinical experience in a similar setting
  • Strong communication and clinical assessment skills
  • Experience working with underserved populations
  • BLS/ACLS certification
  • Must pass background check and drug screening
  • Valid driver's license, insurable, with reliable transportation for local travel

Spanish language skills are a plus but not required.

 

Physical & Environmental Expectations:

  • Ability to lift up to 25 pounds and work in a variety of clinical and mobile settings
  • Frequent use of computers, standing for long periods, and travel within Laramie County
