ROLE SUMMARY
Community Choice Waiver Case Manager empowers individuals with disabilities to address barriers in order to regain, maintain or increase their independence and access the life and community which is most meaningful to them. CM coordinates in-home services and supports to support Medicaid eligible individuals to maintain independent living.
Specific Duties Include:
- Partner with eligible consumers to:
- Provide information and referral regarding disability related resources
- Build a support network of peers with disabilities (encourage peer support)
- Promote self-advocacy and consumer control
- Increase skills for independent living
- Facilitate comprehensive assessment of consumer’s independent living needs
- Assist consumers to establish Independent Living Goals and Plans
- Provide ongoing support as consumers move toward stated goals
- Deliver services in accordance with policies and procedures established by the Wyoming Department of Health, Community Based Services Section
- Maintain accurate and timely consumer records, including case notes, reports, and documentation using various digital platforms
- Coordinate and conduct community outreach and systems advocacy
- Establish and maintain collaborative relationships with community partners and stakeholders
- Advocate for the rights, resources and services that are key to independent living
- Travel throughout designated service area to meet with consumers and other stakeholders.
- Participate in staff training and related conferences
- Other duties as assigned

KNOWLEDGE, SKILLS, & ABILITIES
- Demonstrated understanding of the IL Philosophy – self-determination, self-advocacy, consumer control
- Demonstrated interpersonal skills and ability to connect to people
- Interviewing and facilitation skills
- Ability to plan, implement and document programs and services
- Ability to adhere to programmatic deadlines
- Proficiency in various technology applications including Microsoft Office
QUALIFICATIONS
Required:
- Bachelor’s degree in social services or related field and,
- One year case management or related experience
OR
- Associate degree in social services or related field and,
- 4 years case management experience
Preferred:
- Personal experience with a disability
- Knowledge of disability related issues and resources
- Experience with or knowledge of Centers for Independent Living
